Report an immunization record
Help your patients maintain an integrated immunization record by reporting administered vaccines to Southeast Public Health. Our team will enter the immunization records into Panorama (Ontario’s provincial immunization system).
Reporting electronically
Get set up with electronic immunization reporting.
Email immunization@SoutheastPH.ca with the name of your facility and the names and email addresses of the staff who will be sending the immunization records.
Once you are set up, you can pull an immunization report from your electronic medical record every two to four weeks and upload it to your facility’s secure folder in SharePoint (you will be sent a link).
Reporting by fax
After administering vaccines to eligible high risk patients and school-aged children, fax a High Risk and School Publicly Funded Immunization Program Reporting form to Southeast Public Health at 613-549-0985.
After administering routine vaccines to children (zero to 17 years of age), fax a Childhood vaccines given by physicians form to Southeast Public Health at 613-549-0985.

To request a patient’s immunization record from Southeast Public Health:
