Notice of intent to add additional personal services application
Inspection reports: Southeast Public Health regularly inspects personal service setting premises to ensure they are following regulations and personal service settings guidelines. View businesses that are inspected by Southeast Public Health:
- Hastings and Prince Edward Counties
- Kingston, Frontenac and Lennox & Addington
- Leeds, Grenville, and Lanark Counties
Effective July 1, 2018, all operators intending to operate a personal service setting in Ontario must notify the local Public Health Unit of their intent, as per subsection 3(3) of the Personal Service Settings Regulation 136/18 made under the Health Protection and Promotion Act R.S.O. 1990 Ch.7.
Please be advised, the completed form must be submitted to Southeast Public Health at least 14 days before commencing to provide additional services.
