In Ontario, health-care providers do not automatically report immunizations to public health. Parents and guardians must report their school-aged children’s immunization records to Southeast Public Health to ensure records are up to date and meet the requirements of the Immunization of School Pupils Act.

Tips for finding your immunization records

  • Look for your yellow immunization card at home. Check medical files, baby books, and school or camp forms. Ask your parents or guardian if they still have your childhood records.
  • Contact the clinic(s) where you were immunized (e.g. primary care providers, travel clinics, walk-in clinics).
  • If your health-care provider has retired or moved, you may be able to get help locating records by contacting the College of Physicians and Surgeons at 1-800-268-7096, extension 603.
  • Contact schools, colleges, or universities that you attended.
  • Contact previous employers, or the military if you served.

View your immunization record online

To log in to view your records online you will need your health card. If you need support, or need an Ontario Immunization Identification Number (OIID), please contact Southeast Public Health.

If you cannot access your records online you can request to pick up immunization records in-person at a Southeast Public Health office during regular business hours.
