Steps to prepare for your appointment

1. Print the applicable requisition(s)

You will be told which package to print during your intake appointment.

For your initial PrEP visit

Initial visit laboratory requisition (PDF – available from your clinic)

For your one month follow-up visit

One month laboratory requisitions (PDF – available from your clinic)

For your routine three month follow-up visit

Three month laboratory requisitions (PDF – available from your clinic)

For your yearly in-person visit

Hep C laboratory requisition (PDF – available from your clinic)

2. Fill out the requisition forms

Include your name, date of birth, sex, health card number, address, and phone number on each page.

3. Bring the completed requisitions to LifeLabs for testing
