Application to reopen a recreational camp Hastings and Prince Edward Counties

This is to notify the Medical Officer of Health of my intentions to reopen a recreational camp in accordance with O. Reg. 503/17: Recreational Camps, under Health Protection and Promotion Act, RSO 1990, c. H.7 The personal information on this form is collected under the authority of the Health Protection and Promotion Act and will be used for the administration of Public Health programs. As the person intending to operate the camp, I understand that I will also be considered the operator, unless I designate another individual as the operator at this time.
